◆エイズの病原ウイルスであるHIV（ヒト免疫不全ウイルス）の感染が拡大していま
す。厚生労働省のエイズ動向委員会がまとめた昨年一年間のHIV感染者・エイズ患者
新規報告数は速報値ベースで1114人と、初めて年間１０００人の大台を超えました。
感染経路ではここ数年、男性同士の性行為による感染の報告件数が急カーブの右肩上
がり曲線を描いて増加を続けています。異性間の性感染は横ばいに近いゆるやかな上
昇曲線ですが、報告されていない感染、つまり感染はしているけれども気が付いてい
ない人が増加していることもデータの分析からは推測されています。
◆昨年末に発表された国連合同エイズ計画（UNAIDS)の最新推計では、世界で最も
HIV感染者数の増加率が高いのは中国や日本を含む東アジアでした。地球上で最も遅
くまでHIV感染の拡大を免れていた地域と考えられていた東アジアでも、とうとうエ
イズの流行が本格化してきたのです。危機の到来はもう明白です。
◆そうした中で、我が国のエイズ政策の根幹ともいえる「後天性免疫不全症候群に関
する特定感染症予防指針」（どうしてこう悠長なんでしょうか、エイズ予防指針と略
します）の見直しが進められようとしています。
◆エイズ予防指針は1999年4月1日施行の感染症法（感染症の予防及び感染症の患者に
対する医療に関する法律）に基づき、同年10月に作られました。制定後５年をメドに
見直すことになっており、その見直し作業を行うため、厚生科学審議会感染症部会に
検討会（ワーキンググループ）が作られ、２月９日に初会合が開かれました。５月か
６月には新しい予防指針がまとめられる予定です。
◆ということなのですが、それではいったい予防指針とは何でしょうか。見直しっ
て、それが私にどう関係するの? エイズ対策の現場からさえ、そんな疑問の声も聞
こえてきます。現在の予防指針はどのようにして生まれたのか。それは機能している
のか（この５年間、感染が増え続けているのだから、機能しなかったと考えるべきで
しょう）。していないのだとしたら、なぜ機能しなかったのか。指針の何を見直そう
としているのか。HIV/エイズの流行の現実は５年間でどう変化したのか。予防指針と
いうけれど、じゃあ治療やケアはどうなるのか。個別施策層とは何か。疑問はどんど
ん膨らんでいきます。
◆知らない間に国のエイズ政策が決まっていってしまうのも、あまりといえば、あま
りです。とくにいまは、ちょっと見逃せない事態を迎えている時期でもあります。ま
ずはワーキンググループの委員や5年前の指針策定時の事情に詳しい人、そしてエイ
ズ対策の最前線の現場にいる人たちの報告を聞き、見直しに何を望むのか、はっきり
意思表示をしていくことが大切です。

南アフリカは、1997年から2002年の間にAIDSによる死亡数が57％
上昇したと報告し、間接的であるにしても、この国のエイズ危機に
よる犠牲者の急激な増加を示しました。
アフリカ統計局(Stats SA)は、広く待たれていた全国死亡率研究
による数字を発表し、報告された死亡数は1997年の318,287人から
2002年の499,268人へ跳ね上がったと語りました。
その報告書はまた、この国の人口4500万人の9人に1人が感染して
いると推定されるウイルスに対し、もっとも感染の危険性が高いと
見られている、女性と20〜49歳の年齢で最も急速に死亡が増加し
ていることを示しました。
その研究は、「南アフリカにおけるエイズ危機が中年成人の死亡率
を押し上げているという間接的な証拠を提供する」と、Stats SA
のヘッドPali Lehohlaさんは語りました。
当局は、「公式に届けられた死因の多くは、結核、インフル
エンザや肺炎であり、これらがエイズに起因することを確認する
ことが困難なままだ」と、語りました。
Stats SAは、南アフリカのエイズ危機は死亡率上昇における重要な
因子のようであるが、増加の一部には全人口の増加と死亡統計の
向上が寄与しているかもしれないと語りました。

ニューヨーク──クリントン前米大統領は１１日、エイズウイルス（ＨＩ
Ｖ）に感染した子どもの治療のため、同氏が主宰するクリントン財団が１００
０万ドル（約１０億８０００万円）を拠出すると発表した。今年末までに、世
界１０カ国以上でＨＩＶ感染の子ども１万人に、抗レトロウイルス薬を届ける
としている。
さらに、国連児童基金（ＵＮＩＣＥＦ）と協力してこの計画を推し進め、２
００６年末までには世界で６万人以上の子どもが治療を受けられるよう活動す
る。
クリントン財団は、インドの製薬大手シプラ社の協力を得て、小児用抗レト
ロウイルス薬を通常の半額以下で供給できるようにし、今年の春には中国とド
ミニカ共和国、レソト、ルワンダ、タンザニアへ薬を届ける予定。
クリントン前大統領は記者会見で、「エイズによる死者のうち、６人に１人
は子どもだ。しかし、発展途上国で治療を受けているエイズ患者３０人のう
ち、子どもは１人以下と非常に少ない」と述べ、子どものエイズ治療の必要性
を訴えている。
国連によると、ＨＩＶ感染者は推定３９００万人に達し、このうち２００万
人以上が子どもだという。昨年１年間のエイズによる死者は３００万人で、５
００万人以上が感染した。

［ルサカ ２５日 ロイター］ 世界保健機関（ＷＨＯ）は２５日、世界
でマラリアにより死亡する人は年間１００万人で、その大半がアフリカで死亡
しており、大陸全体で年間にかかる費用は１２０億ドルに上っている、との報
告を発表した。
ＷＨＯの南アフリカ地域担当者は、ザンビアで行われた今年のマラリアデ
ーの催しに訪れた代表団に対し、マラリアは労働力に影響を与えているため、
大陸の発展を妨げている、と指摘。
同担当者は、「マラリアは依然、アフリカにとって重荷となっている病気
のうち、最も大きな割合を占めている。世界のマラリア発症は年間３億５００
０万?５億人と推定され、このうち６０％がアフリカで発症している。またマ
ラリアによる死亡は年間１００万人を超えており、その８０％以上がアフリカ
で死亡している」と述べた。

【ベルリン＝黒沢潤】ドイツの高級車メーカーＢＭＷは、南アフリカ北東部
ロスリンの工場近くに、エイズ（後天性免疫不全症候群）患者向けの診療所を
開設した。こうした試みは南ア国内では珍しく、同国に進出する先進国の企業
が今後、同様の動きを見せるか注目される。
同診療所は、月に４０００?５０００人の患者の治療を行う施設を持つほ
か、エイズ予防教育やカウンセリングなどにも対応できる。
約７０万人が住む同市周辺には、同社従業員（全約３０００人）も多数住
んでいる。同社のナタリー・マイエット医師によれば、従業員８６％の自発的
なエイズ検査の結果、６％がウイルスに感染、同社にとってもウイルス感染は
看過できない問題となっている。同社はこれまでの５年間、従業員に対して、
エイズ治療や予防の面でさまざまな取り組みを実施してきたが、公共福祉の観
点から、こうした取り組みを広く地域社会にまで広げた格好だ。
ＢＭＷ・南アグループの経営担当者、ウォルフガング・シュタドラー氏は
「南アフリカでは、すべてを政府に求めてはならない。民間企業は進んで、エ
イズ問題に取り組む必要がある」と訴えている。

◆2005/06/30 ３百万人の目標達成断念 エイズ治療で国連

【ジュネーブ２９日共同】世界保健機関（ＷＨＯ）と国連合同エ
イズ計画（ＵＮＡＩＤＳ）は２９日、２００５年末までに発展途上
国のエイズウイルス（ＨＩＶ）感染者３００万人に対して抗ウイル
ス薬治療を施す目標は「達成できそうにない」として、０３年末に
設定した目標達成を事実上断念する中間報告を発表した。
報告によると、発展途上国で抗ウイルス薬治療を受けている感染
者数は今月末推計で約１００万人。国連がキャンペーンを始めた０
３年末の推計約４０万人と比較すれば倍以上だが、国連が設定した
中間目標の１６０万人には遠く及ばない。
国連当局者は同日、目標達成を阻んだ要因として、薬品の調達と
配給が円滑に進んでいないことと、治療に当たる医師など人的資源
の不足を挙げた。
国連は、エイズ被害が最も深刻なアフリカ南部で抗ウイルス薬治
療を受ける感染者数が飛躍的に増えるなど、キャンペーンが一定の
効果を挙げていると強調、今後も各国政府が取り組みを強めるよう
求めている。
非政府組織（ＮＧＯ）の国境なき医師団（ＭＳＦ）によると、イ
ンドで安価な「コピー薬」が製造されるようになったため、抗ウイ
ルス薬の１人当たりの年間治療費は５年前の約１万ドル（約１１０
万円）から１５０ドルにまで減少した。
しかし、薬価が各国のエイズ対策費に占める割合は依然として高
く、ブラジル政府はこのほど、知的所有権に関する世界貿易機関
（ＷＴＯ）の合意に基づき、特許料を支払わずに米アボット・ラボ
ラトリーズのエイズ治療薬「カレトラ」を製造すると同社に通告し
た。

◆韓国でアジアのセクシュアル・マイノリティ会議
Dear all-
We are excited to announce a conference entitled "International Dialogue on
Gender, Sexuality, HIV/AIDS and Human Rights: Focus on Asia". ARC
International is working in collaboration with the Korean Sexual-Minority
Culture & Rights Center to present this conference from November 24th -
27th, 2005 in Seoul, South Korea. (A brief description of both sponsoring
organizations is below.) By hosting this year's Dialogue in Seoul, we honour
the historic decision of South Korea to be a signatory to a statement on
sexual orientation at this year's UN Commission on Human Rights, the only
Asian country to do so.
We hope to be able to accommodate approximately 55-60 participants for the
full Dialogue, and higher numbers for those just interested in attending the
pre-conference Human Rights Institute on November 24th (a tentative agenda
is below). The participants will be comprised of stakeholders in the
international arena (NGOs, academics, and funding organizations) from all
geographic regions, particularly those with some experience advancing human
rights regionally and/or internationally. We invite you to apply by
September 30th, 2005 using the form below.
Building on previous consultations in Rio de Janeiro and Geneva, some of the
goals of the Dialogue are to:
・ Conduct specific training on domestic, regional and international
human rights mechanisms;
・ Explore linkages between civil and politicalrights and economic,
social and cultural rights, including through an HIV/AIDS lens;
・ Build NGO capacity, particularly by engagingwith funders and
developing tools for documenting and reporting human rights violations;
・ Engage with UN experts;
・ Identify strategies and concrete measures tosupport the Korean
government's commitment to human rights, and build support with other Asian
countries poised to follow Korea's lead;
・ Raise awareness of proposals for UN reform, and discuss possible
impact for our communities;
・ Explore faith-based and other responses to religious fundamentalism;
・ Identify regional priorities, and develop strategies to assist in
advancing those priorities, with particular focus on the Asia region.
In terms of funding, our priority is to attempt to assist selected delegates
facing barriers to participation, e.g. those from Eastern Europe, Asia,
Africa and Latin America. "Assisting" might mean direct funding through us,
or facilitating individual delegates to approach their own regional funding
sources. We are still in the process of fundraising for conference
activities and interpretation/translation, and any assistance that
individuals and organizations can provide, including letters of support,
would be most appreciated.

朝日新聞ニュース速報
「薬が効かない薬剤耐性のエイズウイルス（ＨＩＶ）が国内でも広がっていることが、厚生労働省研究班による全国的な調査で明らかになった。新たな感染者の約５％で、耐性ウイルスが見つかった。欧米の１０〜２０％よりは低率だが、本格的な感染拡大が心配される。１日、熊本市で開催中の日本エイズ学会で発表された。
研究班は、ＨＩＶ感染者の治療にあたる全国約３０の医療機関で、０３〜０４年に新たに感染が判明した５７５人（国内新規感染判明者の約３割）を対象に調べた。その結果、３１人（５．４％）でウイルスの遺伝子配列が薬剤耐性に変異していた。
国内では約２０種の抗ＨＩＶ薬が使われており、通常このうち３種を組み合わせてウイルスの増殖を抑える。耐性ウイルスは突然変異で生じ、薬の飲み忘れや治療の中断などで増えると考えられている。早ければ、治療開始から数週間で薬が効かないウイルスが出現することもある。別の人がこの耐性ウイルスに感染すると、使える薬の選択肢が狭まり、治療が非常に難しくなる。
研究班長の杉浦亙・国立感染症研究所エイズ研究センター第２研究グループ長は「欧米諸国よりは低かったが、早急に耐性ウイルスの検査態勢をより充実し、感染の広がりを注意深く監視していく必要がある」といっている。」[2005-12-02-08:10]（全文）
